Forms & Applications

As an NMU Broker, you can access the online quoting system, and have access to your broker’s activity, billing inquiry, and claims inquiry 24/7. However, there may be times when you want to print off a copy of our Quote Request Form. This form contains a list of questions that need to be answered in order for us to provide you with a quote. You will see the same questions on our website for online quoting and will be asked these questions by our underwriters should you decide to call us direct.

The answers to these questions will allow us to give you an accurate quote and increase your credibility to your customer. Here's why. Some of our competitors take a few pieces of information at the onset only to provide an "indication" of the premium. It may take less time; however, the details not provided could have a drastic negative effect on your premium in the end. Our approach allows you to confidently provide a quote to your customer that we will honor, as long as the information you provided is accurate and does not change, should your customer decide to bind. It provides credibility for both of us and establishes a trusting relationship between you and your customer.

A signed and completed Yacht Application will be required prior to issuance if we agree to offer coverage to your customer. We have provided a blank application for you. However, once coverage is approved and we agree to offer coverage, we will be able to provide you with a completed application ready for your customer’s signature. All they have to do is check it for accuracy and sign and return. Once the application is received and approved, we will contact you and review our procedure for policy issuance.

If you wish to review our Sample Policy Form, we have provided a copy for you. It is our most recent form and is only applicable for quotes and policies created on the dates indicated.
